I have a 22" pool with a perfect fit outside to outside as well. It is also important to consider what distance the hole is from the pool edge at the back end. Due to the lip being 22", I cannot slide the ladder down as far as I would like without inside rubbing on the lip (edge). It is not a significant problem for us, but for someone with high hips and a large frame, this would cause difficulty in using the steps. There is no problem with this ladder if your pool edge is less than 22" from the back of the gap.
No, the anchors and chrome covers do not come with it. It is beneficial to already have them or to purchase them online and install them. My anchors were already installed when I got it as a replacement.

It seems that the plastic for the top step has become weakened by the sun since the first step barely rises above the water.
