According to my understanding, that depends entirely on the dog. There is no harm done to the dog by this. The leash is attached under their nose and, thus, pulls their head down so that they cannot see ahead, so if they pull they stop pulling in order to raise their head. It is not on my list to stop using. A dog that pulls with this collar is less likely to choke than with a normal collar.
There is absolutely no doubt that you have to attach the head harness to a collar or harness with an appropriate safety line! To find a safety line for head harness, just do a Google search. When your dog slips out of the head harness, this is used to keep him from falling off. now that your leash is attached to your collar/harness, and your head harness is loose, you should attach it to the collar/harness I breathe. If possible, choose a safety line that is long enough not to restrict the movement of your dog's head.
