Everything looks exactly as it does in the picture. To spread enough to cover the stethoscope and to close it, you have to work rather hard. A little bit of a pain, but Since my uniform is so drab, it is nice to accessorize in any way that I can!.

I am in love with them. They are snug enough on my Littmann Cardiology IV that they stay in place when I use them, and there is no interference with the acoustics of the stethoscope. Y'all wonder why I haven't blinged out my stethoscope before now?.
