Yes, it can be used outside. To make your electrical box flush with the block's face, you'll need to add an extension. A hole saw or a utility knife are used to cut a hole in the block.

Yes, that is correct.

Is it possible to use this with pre-existing wood siding?

It has a flange for nailing. It must first be installed, then the siding must overlap the flange and the face must slip over and click into place.

Because my electrical box is flush with the sheathing, what size electrical box extender is required?

They are as follows: 1. 5" in depth I'm using a round metal ceiling fan box from Home Depot, but I'm also adding a fixture where there isn't one already.

I covered the block with Hardie Board and topped it with a galvanized drip edge. The frame was tossed in the air.

These were ideal for mounting behind our new exterior lighting after we redid the siding on the house. They are very reasonably priced, arrive quickly, are simple to install, and they are very attractive.
