Only the blower fan needs to be changed.
I have mine hooked up to a wall switch. To replace the standard toggle light switch with a dimmer switch, I bought an inexpensive one online. I've got bingo. You can adjust the fan speed! I love how it works.
This product does come with instructions, but they are extremely limited in scope.

The Majestic unit in my new house came with a smaller [12"] fireplace fan, so I purchased this to replace it. Despite being an effective heater, my old fan was so loud while it was running that I couldn't hear the TV unless I turned it up a lot. This new fan is just as loud as the old one but has a larger blade area so it seems to pump out more air. have a fan speed control unit and I set it to run slower [and presumably quieter] and in fact it has worked great. As I didn't want a control knob on the wall instead of a simple on/off switch, I got an inline type controller and mounted it under the fireplace where the fan plugs into the wall. The controller has its noise level preset so it won't be distracting.
I like it and you can still hear it but it's just a low background noise. Great price for a powerful fan and if you like to hang out at airports this is for you, if not purchase a controller and adjust Regardless of the way it's bought, it's a great deal. There is a loud metallic rubbing noise, but the wind noise is very low. This was not the case It was as if there was a metallic rubbing sound at all speeds. The answer is yes. There is movement of air with this blower. Because of this, it is also very loud. If you talk right next to the fireplace that the unit is installed under, it won't drown your conversation out, but it is impossible to avoid hearing the noise. As a result of getting this unit and installing it, our fireplace has been more efficient at heating the house, both in terms of the heat they pump into the living room as well as the amount of time that it takes. My fireplace was perfectly fitted with this product and it used all of my existing cables. As an alternative to the velcro tape this unit uses, my previous blower had strong magnets as a mounting mechanism, and I preferred those over the velcro tape used here, but the extra airflow is worth both the extra sound and the flexibility in mounting.

During the winter, I heat my house with a 1985 wood stove. My original fan became extremely noisy, so I searched for a replacement. The bearings on the original fan failed (lots of vibration at high speeds). Although the blower is the same size as my old blower, the only difference is that it has 160 CFMs to it instead of 120. Originals have 180 CFM, but most people don't run the blowers at their full power, so this unit works just as well. It is much cheaper as well.
After two weeks, the machine was very loud, but then shut off and stopped all together. We smelled burning electric/plastic fumes after it stopped working immediately. Installing is a pain in the bum. Thank goodness we were at home just in case it decided to continue "working" while the house burned down! My lovely Father in Law installed this for me, but he said it was excellent quality, fit perfectly, and was easy to install. I also liked how easy it was I also like how well it was packaged for protection. We have been very happy with the unit and it is heating up the room very quickly.
