I will let my son hold it himself when he uses it. I don't think it's great that he can twist off the top and wants to eat just the tip of the fruit. It is therefore always a good idea to supervise young ones when In this way, he's stimulating his mouth and tongue so that he can hopefully start talking.
This tool is my 3 year old's preferred method of massage over manual massage he even sticks it in his mouth to use it.
