The switch has been in my possession for over 3 years now. My memory is fuzzy as to whether or not the plate comes with it. It is my guess that it is not.

The fan power wire is now connected to this controller which is responsible for managing the electrical power going to the fan. I removed the remote control circuitry from the ceiling fan and connected it to the controller. The three speeds are great, but it'd be nice to have an off setting. Furthermore, the fan can also be reversed by turning the fan quickly on and off for reasons I'm not sure about. A few times I can get the fan to reverse direction if I do that multiple times. The switch might not be able to have that property. I think it must belong to the fan. With separate wires and two switches in a junction box, I started with what I had A fan and a light should be mounted on each side of the fan. The voltage was wired to both switches, so either switch would power an electronic controller that was able to control both the fan and light, as well as set the intensity of the light. It was awkward and confusing to use the remote to turn on one switch and not the other, as you could not switch it on without using the remote. The fan and light were each wired to a separate switch instead of pulling out the control unit from the ceiling fan. We had issues with our previous dimmers as they dimmed the light as the fan speed increased. A call to Amazon immediately followed by a 3 way call to the manufacturers revealed the light was not these and the LED light was very bad and worked only in one switch position! Apparently LED lights should not be used with these dimmers so be aware of that.
It works well with our ceiling fans, we have 3 of them installed. Although we had remote controls for the fans, they interfered with the other fans in other rooms. I installed these switches and the issue has been resolved. An added control on the wall is less aesthetically pleasing than a clean wall mount.

There was only one issue I encountered, and that was getting the light switch, rheostat, and faceplate to work properly. I finally got it, but it took a bit of effort. What an amazing pair of shoes. I originally bought one from Home Depot, and each switch cost me $4 more. In addition to making sure it worked and was quiet, we also wanted to make certain this particular model was available. There is no room for the motor in the electrical box, so you will need to tuck wires "creatively", but we fit this and a rocker switch with caps on all connections by tucking wires out of sight. I highly recommend these fan dimmer if you are in need of one. The fans are silent (although some motors will hum slightly from the switch itself, not just the fan). I highly recommend this product to anyone who wants a quiet yet wonderful product.
This is a very good product. The transitions between phases are very smooth, which is why I purchased it to replace a fan switch that was rather hard to cycle between.
