By default, the remotes are set to 'all zeros,' and most people only change them if there are interference issues; try using it right out of the box. If that doesn't work, you can try changing the switches' codes (there are 256, so it's not impossible) or open up the fan's shroud to see what the controller module has set.
I don't have access to the remote right now to provide identification, but it was functionally identical to the unit that was replaced.
It came with the holster to attach it to the wall when I purchased mine from Ceiling Fan Parts Superstore (Amazon seller). The holster is nothing more than a small, thin piece of plastic that screws into the wall.
Open up that company, ask for parts, and they should have the correct remote. Google what type of fan you have. Richard

I'm guessing this model was purchased around the same time as the house, which was eight years ago. I've included a photo of the original remote control as well. The original remote had stopped dimming and brightening the fixture's up and down lights. This replacement remote was a gamble for me; and was ecstatic to discover that it performs admirably. With a single click of the appropriate button, it turns the up and down lights on and off, and if you hold that button down, it turns them off. It'll change the brightness of the light it's in charge of. The fan control functioned flawlessly as well. Low/Medium/High, as well as OFF, are all well-adjusted. When the replacement works better than the original, it's always a happy (and unexpected) day: ) When programming the replacement remote, make sure the four (very tiny) switches (under the battery cover) are in the same position as the original remote's four switches (also under the battery cover).
Every description I read said that in order for it to work, you must get a "identical looking" remote control to the one you already have. However, I was unable to locate one. I found this to be the most similar to my own. Although it appears to be identical, it isn't. Surprise, surprise, it WORKED LIKE MAGIC! I'm in a great mood! There's no need to go out and buy generics or deal with the wiring in the ceiling! Customer is overjoyed!.

1 If you're not sure if you have the right remote, don't order. My original remote was slow from the start, but when it stopped working years later, I replaced it with this one, which is quick and responsive to the fan and lights. I'm glad I bought it.
Our old Hampton Bay remote had finally given up the ghost; it was in pretty bad shape, with the battery cover missing and numerous trips to the floor or behind the bed. This one is far superior to the one that came with our fan. I assume it works with multiple models or houses with multiple fans because it has internal dip switches, but it worked for us right out of the box. The sleeve, or holster, as you may refer to it, was a welcome addition. There will be no more misplacing your belongings behind the bed! I'm not sure how you could get a better product for the money, and I was skeptical that this one would work given its cost, but I have no regrets. It's still going strong, and all of the buttons work and do their jobs.

It's not a "universal" remote, but it's a perfect match. Their description of the product was accurate. This is a new remote to replace the one I had before. As soon as I inserted the battery, it began to work! Colored buttons are appealing to me because they make it easier to remember what each button does. My ceiling fan has a great remote control. I have two ceiling fans that I can control with a remote. I'm going to get a new one soon just for the convenience of having a remote control. It's so nice to be able to control the fan speeds and lighting without having to get out of bed when I'm exhausted. This has been especially useful when I've been sick and unable to leave the house.
