The one I have does not have a padded ear guard. They are also not on the other book I have. If it is really cold under the helmet I will wear a balaclava for warmth, if the water is really cold I will not use ear guards. When I wear a large or even a XL, I have no problem. Their size chart indicated I should have been a large, but it wasn't snug at all, and there was no way to adjust it for this model. In the case of a large it probably would have been adjustable to my head if there was a "dial". It looked like the cutouts for ears were well placed, so it would be comfortable if it were the right size.
