It isn't clear to me what you mean by a medium fit. I'm 5'7" with a 33 waist and a medium is perfect for me. I would have liked a denser pocket, but otherwise, it is a good value and well made. There are some high-end brands that copy their designs.
As far as size goes, I chose large and I am 250 5'10" with monster thighs and a 36 waist, but I would probably recommend one size up, the quad bands are a bit tight when my legs are bent! I went on my first trainer ride with them tonight - what a great experience! I just wanted to give you a quick 30 minute high-five It was a hard workout, so it isn't the perfect test, but it's close enough. Chamois that's super comfortable to wear. I previously had issues getting my bike seat on the trainer to stay on, but these bib shorts completely eliminated those concerns. They are well worth the money. I use them quite often on the indoor bike set-up (road bike + smart trainer), and I as well use them outside. The shoes have been tested most weeks by riding 5 or more times for a total of 6+ hours a week (though I haven't worn them for more than 3 hours at a time). After machine washing (front loader) with cold water and tumble drying on "very low," they have shown no signs of fading or pilling. However, as it is, they have been washed dozens and dozens of times and still fit as they did when new. As they are, I can't imagine they would last as long if they were hand washed and line dried. In my case, I'm 6'2" and about 220 lbs (ish), so the XXL fits just right. The price is the first thing we like, but there are a lot of other advantages as well. There was a specific reason I wanted pockets on the sides, and this fits the bill perfectly. I ordered enough of these and other pockets to get through a week of riding. 1) the fabric is good enough, the shoulder straps are comfortable and the pad gives a great fit. I'm sure more expensive bibs are more comfortable oder more durable, but for the money and the amount of riding I do, these are perfect! There's no way I'm wearing shorts while I've got a beer belly.
