Sadly, there is no carrying case that comes with the item.
In my experience, it does, but I have used it multiple times and was never much of a fan of the tilt. I found that it works better open-side down and with the open lip facing you, so that you can easily drop one pair and pick up another.

I can fit my percussion instruments comfortably in it.
What I was looking for was exactly what you provided.
The foam needs to be Velcroed to the table, otherwise it will fall off.
There's a danger with this purchase I made elsewhere. While it appears to be solid, it is not. There was ample space to hold my mixer and small amp for home recording studios, but you have to insert a channel wrench to tighten the tilt bolt to hold the table in place. Having your hands too tight and putting a couple of pounds on the backside will cause the camera to flip over and all your gear will go flying. Changing the tilt by putting your hand on it or bumping it is simple.
