It will work as long as the pin piece is able to pierce the vest and screw into the main unit. I'm a firefighter, and this piece is worn on my right shoulder, with the microphone cord running around my neck and down to my HT in my jacket. Just keep in mind that it will leave a small hole in whatever you use it to attach to. I hope this information is useful.
During an event, I wear it high on my collar on what I'm wearing, which is usually my turnouts or just a hi vis vest. Because I have some hearing loss, this brings the speaker closer to my ear when there is noise. It's just that when you remove the strap, you forget that the radio isn't attached to it, which happens frequently.
Phillip is correct, and I concur. Drilling a hole first is the simplest option; the pin is threaded, so as long as the thickness of the leather does not prevent the screw from engaging the thread, it should work.
That's what I think. Vertically is how I use mine. It's simply a retractable device that allows you to place your microphone back where you want it. Mine is extremely effective.

I came across these by chance over ten years ago and decided to try them out on my bunker coat. I wear my radio under my coat and it was a pain to deal with the mic swinging back and forth or falling back inside my coat if I left it hanging out the closure. I hook the Mic through the elastic loop as shown in the packaging and attach it to the Mic loop on my jacket. It holds my microphone in place and allows me to use it while wearing structural gloves.
UPDATE. . . I contacted the business directly and was directed to the manager. He expressed regret for the error and offered to send a replacement stud at no cost. I've been using it for the past two months and in multiple fires with no problems!! Although the initial problem was most likely due to a manufacturing flaw, the customer service was outstanding! I'm a firefighter, so my flashlight strap is attached to my bunker coat. It's lasted a long time and continues to perform admirably. This product is fantastic because it allows me to keep my microphone where I want it.
This is perfect for my work radio. I'm a firefighter, and when I'm in bunker gear, I keep my radio in an exterior pocket, which allows me to keep my microphone in a convenient location while still being able to move to my voice amplifier as needed to get transmissions out on the fireground.
For my microphone, this was exactly what I needed. Yes, you punch it through your material/uniform, and it's simple to use and perfect. So my shirt wouldn't get all holy and tear, I ironed on two small black patches, one inside the other out. The quality of the product is outstanding.
This was purchased for my turnouts (firefighting equipment), but it was missing the pin that would allow it to be attached. Otherwise, it appears to be a good fit.
As I was installing it, the pin and thread broke, and I see that others have experienced the same issue. It was a complete waste of my money.
