It is usually measured from the leather start at the buckle end to the third hole from the tip end of the belt. Usually you would order 1-1/2 lengths based on the measurements of your 4yo. So, if your 4yo measured 22", then it won't be too long, but it might be a little too short. To fit perfectly, your waist measurement needs to be 2 inches larger than your hip measurement. There is another better way to pick a desired size, if you have an existing belt that fits well, then measure from the buckle end (excluding buckle) to the hole currently used.
