Unfortunately, I am unable to tell you which model number is correct. In many cases, it is not necessary as this part will fit any Kohler toilets that come in a single piece and have a tank and seat as a single piece. Please be careful with the installation instructions as they are precise and correct. My previous valve had a broken notch and it was very difficult to tell since there is a hidden notch beneath every notch, causing the tank to leak water slowly into the toilet and you would not see anything wrong. There is some force required to install, but if you are pushing too hard and the valve does not turn, then something is wrong and you may be damaging the notches by turning the valve in the Just apply a little more dish soap to the rubber gasket and rotate the gasket in the opposite direction in that case. You might be pushing against these notches when welding the tank together if you find the underside is adorned with small indentations in the ceramic.
My understanding is that it will, but you would be better off contacting Kohler directly to be absolutely certain.

There were no stores in my area that sold this part. The only parts they have are for two The toilets come in three pieces. With the seal in place, it was somewhat difficult to get it to fit. We were able to twist the part into only certain areas of the part since certain areas had little space. With the other seal I used, I was able to twist the part slightly further, which helped the part stay put.

When you gotta have it, that's where you go. They were unavailable at Lowe's and Ace as well. Purchase of a new toilet with normal valves would be cheaper.
Unless you have a shallow sink, this is too much for the kohler cimarron k3489.
