I started with an XL when I was 254 pounds and a waist size 44, and I'm 5'7, and now I'm down to a L and I'm 209 pounds and a waist size 36, and I'm thinking about going down to a M soon because it doesn't feel as tight as it used to. I would recommend starting with a L or you may be able to start with a M depending on how tight you want it to fit on you.
