In multiple occasions with multiple users, it crashed the Scantron brand scanning machine at my school (before using this form, I don't remember how I was about to know this-- Seeing your phone or fax machine crash was like watching your phone or fax machine go wrong). This error only occurs on this form and not any other forms I have used (not another Scantron form, or a third party form I have used in the past). Due to its inability to be used, it is of very little value regardless of its The Ph. D. program.
