The picture seems to be mounted on the wall, but how? As shown, it appears to be just floating because it does not appear to be attached with screws or strong adhesive. ?

An imprint of the mounting plate is visible on the rear of this model. Wall screws are attached to the imprints and the screws are drilled into the wall.

Does the hardware for hanging the glass on the wall come with it?

There are all the screws and hardware you need to hang it
