If the phase outputs will be connected to a voltage source, there must be a pull-up resistor (1kOhm in my case) connected to the signal source. Initially I thought it was broken, but I read about another very similar unit on Amazon that turns out to be a sinking NPN output.

The encoder worked perfectly with an Arduino Leonardo. It output a nice and clean signal as it was fed by 5V from the Arduino. You need to declare the pins from the encoder as input_pullup in Arduino or else the encoder will just read LOW whenever it is powered. It comes with 3 M3 screws, the same ones pictured. There was no mention of this in the item description, so I thought that I would The product is excellent.
