You should be fine in a medium, but if you notice that your belly is expanding significantly, it's not a bad idea to size up to ensure that you can wear it later in your pregnancy. However, most maternity clothing sizes correspond to your pre-pregnancy measurements!
I'd recommend sizing up to a large so you're comfortable, but the material is stretchy. I'm wearing an XL and it still fits well despite the extra room, so I'm glad I went up a size.

I went with the small size. I adore the softness of the fabric. I usually enjoy shopping, but it's been difficult for me to find clothes that are comfortable for me since I became pregnant. For my upcoming baby shower, I purchased a number of dresses. At 28 weeks, I was 5'3" and 134 pounds. Pre-pregnancy, I weighed 114 pounds and was a petite woman. Another dress appeals to me, so it's a choice between this and another. I will, however, keep this dress regardless of the outcome because I have a couple of other baby showers to attend in the near future.
BUY IT NOW. I bought this dress when I was 35 weeks pregnant (194 pounds) to wear to my sister's wedding in a few weeks. The only real difference between the sizes was length, so I bought a large in navy and an xl in maroon to see which one fit better and which color I preferred. I also think the navy in large is more flattering than the maroon in xl, so I'm keeping it. I don't think the dress would hold up in the wash or for multiple wears, but I only plan on wearing it once for a wedding, so I'm not going to find out. This thing is INCREDIBLY soft and comfortable to wear, which, if you're in your last month of pregnancy, you know how uncomfortable any type of clothing can be. Finding something like that is truly what gives this 5 stars.
It looks great on you. I'm normally 150 pounds and 5'7" tall. I wore it at 37 weeks, weighing nearly 180 pounds, and it fit perfectly. If you're debating whether or not to purchase it. DO IT!.
