It seems XL is the right size as I am 4'10" and 240 pounds. I can fit comfortably and there isn't much extra room.
You can sit on the flap just by pulling the two sides apart. However, I find even the biggest flap too small for me. Unless I get my other parts through the toilet seat at the same time, there will not be enough room to pee while I am tucking the material between the toilet seat and my backside. The 2xl is the right size for me, as I am 6'2" and 275lbs.

Having bought these for my husband, he is 5'10 and 160lb and they fit him just fine. I think they may be too long for your brother as well. It might help if you read this.

We bought these for the husband since his other pair began to lose their shape after a couple of seasons. It is the utter softness that first catches the eye. Husband says they are warm and comfortable, but he doesn't overheat while working in the garage while wearing So far, they have been washed a few times and are still in good condition The new feeling feels good. If you are looking for a more comfortable fit and feel, these are the best The lighter brand will allow you to wear it all day.
The material is of excellent quality. The thermal union suit is made of a cotton blend and is double layered. Winter nights and days can be extremely cold with this jacket. In addition to his height, my husband weighs 245 pounds and is 6'1". The comfort, the quality, and the warmth are what make him happy. The thermals are quite thin, but they run small, so I recommend getting a size up. I ordered the 2X and wish they were available in a larger size. Based on the reviews, I would highly recommend these thermals, but recommend that Since they are of good quality, my husband is keeping them despite the small size.
The weather is not as cold as it will be for some time, so my first "union suit" has not yet been thoroughly tested. It fits me well, though, and I'm mostly pleased with that. I have a bit of a belly compared to the rest of my body, and so my belly sits a little out of place and the buttons jut out a bit, leaving slight gaps between buttons, but it fits well and is comfortable overall. In addition, they are easy for me to get in and out of, even with stiff inflamed joints etc. The rear flap is even within my reach, so I can access it whenever I need to use the restroom without removing the suit. I find the arms to be short and my arms are not particularly long, but it is a plus as it keeps the sleeves from showing outside your shirt. The cuffs fit snugly to keep the sleeves in place, and keeps the air flow from blowing out while donning your shirt. It's been a little while since I've worked hard on the suit. So far so good for the wicking properties, but I haven't tested that much. My union suit has excellent mobility, plenty of room for action, and no seams to restrict the freedom of movement. I ordered another brand/style of union suit at the same time, and it did not arrive until one week later. Other suits are intended for severe weather, or longer wear situations, and at twice the price, I expect they will be more comfortable however, I plan to get another one that is similar to this one to have one washable while I wear another, and this will suit me quite well for most situations. Bib overalls are something I usually wear because of the discomfort of tightly fitting belts, and these meet that comfort requirement very well because they do not have a tight under garment elastic waist, and that makes them great for me as well.
When I arrived, I found the fit to be perfect. Sadly, it shrank so much after the first wash (cold water, lowest heat setting for dryer) that I was no longer able to wear it. To account for shrinkage, when you order, I recommend ordering 1 (one) or even 2 (two) sizes up from your actual size.
The COLORFULLEAF men's union suit was much softer, fit better, and had no sagging bottom or too short legs compared to other brands. My sleeping bag is a medium-weight garment that might be nice as an underlayer, but I use it as a blanket. These are the best pajamas I've ever worn. The only down side is that the buttons tend to open up between one another. As a result, the underwear could show through, so be careful and wear underwear if you want to avoid being.
This is what I ordered for my boyfriend, and he absolutely loves it. I really like how soft they are and how comfortable they are. It fits him perfectly, so there is no need to size up or down. He is 5'5" and they are long (he has short legs, so that's what I would expect). There is a button on the backside ( ) and on all four sides. The gloves are about the thickness of a sweatshirt, and we live in Arizona, so we don't need anything too bulky. The straps are too tight and he does not want to take them off! I love them so much. I appreciate it.
My legs are good (around 32-36" I have a waist that is a great size for my height (around 38 inches), but the sleeves are just 3/4 the length that they should be (I'm a 34-36 shirt). I wear a size 35). One of the other issues with the device is that the many buttons are small and hard to use. I can still keep the thermostat down a few degrees in cold weather and stay warm in the house.
The fabric is very comfortable and the fit is excellent. Attached buttons are in good condition. You wear the dress no matter which way you want to wear it the cat may be slimmer than you expect. The fit is perfect, the crotch is not too short.
You can wear it left or right.
