I've only attempted it once and failed, but I was also attempting to plug a tire with a large bolt stuck in it at a 45-degree angle.
There are thin steel wires in the thread area, so the plug will stay in place for a long time. It's not safe to plug the side of the tire because it's all rubber, but these are high-quality plugs in general.

Avoid the ones at your local auto parts store; I use them on my personal car and they always last until the end of the tire's life. This is a far superior product, and it is well worth the money. I also use it for customers who can't afford to replace a tire and need some time to save up. I never recommend a repair unless the customer understands the risks involved, which means the tire was not dismounted and checked for sidewall damage (which can be difficult to see from the outside). You're not going to be able to use this on a flat tire. Your tire will either blow up or have bubbles in it at this point. But, hey, at the very least, the plug will not leak =).
I've got enough for a lot of flats now. I hope I won't need them, but as a truck driver, flat tires are unavoidable, and I fix them on the road to avoid having to call for roadside assistance. I've used them to plug a hole the size of your index finger; granted, I was desperate at the time, so I figured I'd use four of them at once; I wasn't expecting them to hold, but they did, and I was able to continue with my deliveries. They were the ones who hung on the longest.

They were purchased after I had a flat tire and the roadside service guy had used one and recommended them. I think they're fantastic, and I like that they're considered permanent. I just got the plugs (at a great price) because I already had the tools, but if you don't get the kit, they come with T handles, which are required to push the plug in. The kit is a little pricey, but when you consider that a patch at the shop costs $25, you'll save money if you use it twice.
The blackjack plugging kits are the best for do-it-yourselfers, but the plugs are difficult to come by over the counter, so I was thrilled to find these online, which are of the same high quality as the originals.
Black Jack is, in my opinion, the best tire repair product available.
They become brittle as they age, which is very important.
