How many are required for a complete set in the back?

Two of each item are included in this kit. One set of bolts, washers, and nuts for each side.

Is there a template included in this kit?

Yes. It's a piece of paper that's been printed on. I'd glue the paper template to a piece of cardboard to make it a little stiffer and easier to work with.

How long is the bolt and how wide is it?
