Why is there only one 90 boot in the picture? Does this come with a straight coupling?

The intercooler is where the 90-degree coupler connects. That's the only time a 90° connection is required. The connection at the top is straight. There are no additional parts or modifications required for installation with this kit.

Is it possible to get both boots for this price?

Yes, both of the boots are included.

Does this pair well with a pair of Bolth boots?

Everything you need is included, but keep in mind that this is for a tuned truck, not a stock truck!

Is this kit a good substitute for mass air? And does it take the place of the inner cooler's bottom part?

The egr butterfly and the plastic pipe that leads to the cooler are the only things that are removed. It does not take the place of mass. The inner cooler is not replaced in any way. I noticed a better throttle response and less turbo lag after removing the valve and replacing it with a better charge pipe.
