My daughter wears a big kid size 3 and it fits perfectly in the shoe, so it could work for a size 2 as well.
They do, in fact, sit on top of the insole of the shoe. There's no need to go up a size in the shoe. My son found them to be very effective.

My 11-year-old son suffers from Sever's disease and plays soccer all year. Long periods of time in the cleats irritated his heels, and he struggled to get through practices and games. After reading the reviews, we decided to give these heel cups a try and are very pleased with them. After wearing these in his cleats and any other shoe for about a month, his heels are no longer bothering him. It wasn't a miracle cure, but it did the trick. It took at least a week of use before the pain was significantly reduced, but he noticed a difference right away, and his heels are no longer bothering him. The cups slipping into his shoes and moving during soccer or basketball was our biggest concern. He did say that they moved a little the first time he brought them in for soccer, but that they haven't had any problems since then. Because he forgets to switch the heels between his regular sneakers, basketball sneakers, and soccer cleats, we're thinking about getting a few more pairs. We'll order more of these if we succeed. Everyone is different, so your results may vary, but they worked for my son, who had heel pain for at least a year and a half. br>June 2019 update. My son continues to benefit from the use of insoles. He is a very active person, and the insoles last for four years. 6 months. We were in desperate need of replacement insoles, so we placed an order for more. They lose their tackiness after a few months of active use, and the gel material begins to come apart; however, I don't consider this a drawback because he destroys shoes like Godzilla destroys Tokyo. He not only beats them up, but he also plays and practices on fake grass. We have to take them off and shake out rubber turf pellets before he puts them on because the black rubber pellets get into his shoes and reduce the life of the heel cups. Pellets frequently stick to the cup or get stuck in the creases. Again. That is not a criticism of the product; rather, it is an example of reality. My son no longer has heel pain, but we have discovered that we may need to purchase more of these two or three times a year. We don't mind because it helps him with his Sever's. br>Updated on July 20, 2021. With the Sever's, my son still gets some heel pain now and then, but we're still using these heel cups, and his flare-ups are much less frequent than they were when he first started having heel pain a few years ago. We've discovered that ordering these heel cups for every pair of shoes he wears on a regular basis is the best way to go. If we leave the cups in a pair of shoes and don't move them, they appear to last longer. After about four hours, We usually replace them after 8 months of use, depending on how much they have been used. Rain-soaked soccer cleats, soaked shoes, and rubber pellets require more frequent replacement. The tackiness on the bottom of the cup starts to fade after about 6 months of being abused by soccer cleats, and the dark blue rubber horseshoe on the bottom begins to separate. For the abuse they take, I think that's a good quality. I've never had a problem with the gel ripping, shredding, or pilling up after wearing multiple pairs of these.
It bothered her in the morning as she tried to get through sports and other activities. We gave her some rest, told her not to jump on the trampoline, and told her to ice it. After that, we bought a two-pack of these, and she hasn't complained in three days. She keeps one pair in her soccer cleats, which she will transition to her snowboard boots, and the other pair in her regular shoes. Definitely a purchase worth making!.
Sever's Disease developed in my very active grandson, causing swelling and pain where the tendons connect to the growing part of his heel. He's been having trouble walking for the past two weeks, so I've had to put him in a shopping cart. He put a gel pad in his shoe for his injured heel today, and he's walking almost normally now, with less pain. I had not anticipated it working so well. br>br>Additional information: At bedtime, he refused to remove the shoe with the gel pad, and slept the entire night in it.
Our son has had Severs on and off for a long time, but once baseball season began last month, he began to notice the pain in his heels again. We decided to try these in addition to the stretches recommended by the doctor. I was concerned that they would interfere with the fit of his cleats, but this was not the case. They were a perfect fit. He said they made a big difference in last night's game! He wears a size 6 shoe for reference. These might be too small for a teen who wears a size 7 or 8, so you'll have to switch to adult heel cups.
I wasn't sure if these were going to be worthwhile. Indoor soccer is a sport that an 8-year-old can participate in. One of her heels is hurting, and she isn't putting her full weight on it. Severs is most likely. So I bought these and wore them with a pair of regular sneakers and my indoor soccer shoes. She couldn't tell the difference at first, so she took it out of the soccer shoe and put it back on, saying, "Oh yeah, it's much better with it in the shoe," so they're back in. When she walks, she still favors her left foot a little, but it's getting better. I'm not sure how long they'll be able to hold up - They don't appear to have much going for them, but there's only one more game of indoor soccer left for the season, so if they can make it through the next week, it'll be fine.
