You can open the lid if it has been manually closed, but you will need a flat blade screwdriver to pry it open. For wall mounting, I recommend using a bracket. is possible for the contents to leak if it is knocked over, but not likely.

Is the opening wide enough to fit a catheter sleeve through?

If you dump the butterfly needle completely in there, then you'll be able to thread it
