It has a 3rd prong and is standard AC 115v. I believe it employs a 50-percentage-point strategy. Like a mini fridge, it runs at 60 Hz. Ours is plugged into a standard outlet and has worked perfectly for us.
That would be determined by the coil size you were using. 400 sheets of 20lb paper can be held in a 50 mm coil. That is the largest coil we have on hand.
I just ran the machine through its paces with two sheets of 60 paper and 12 sheets of 20 paper, and it performed admirably. I'm afraid I don't have any 28 paper to test with right now.

We bought it for school use and like it because it's simple and quick to use. It's ideal for schools and/or small to midsize print shops because it's simple to use and quick.
