When you put this together, is there a way for it to be loosen? How did I manage to do it by accident?

While it is possible to loosen it, it compromises the locking mechanism.

Don't waste your money on cable ties, use a hose clamp instead

It was impossible to tie the ties tightly. A hose clamp is much more effective than ties since they contain a lock and will never loosen. But the ties do lock and will not come loose, whereas a stainless steel hose clamp will. The price is also lower.

There was another set of 100 ties I bought for the same price that were also off-brand ties that were just as good

Make sure you're not too tight

Regardless of the way you try to pull them tight, they'll never be as tight as a plastic zip tie. Same result with two different brands of tie. This is the second time I have tried this.

The one who worked was also a student. I tried to pull them loose, but they didn't budge no matter how hard I pulled them. However, I can never get them to be tight enough to hold anything in place, no matter what I do. I got a refund and went with hose clamps instead. The prices are lower and the product works.
