I have a 3 in diameter x pipe, will this still fit?

I don't think so. The stock exhaust has a rating of 2. The over axle pipes are 75" long as well. They were easy to install on my stock h-frame. pipe - also 2. 75".

What is the procedure for putting on the heat sleeves? Is it true that they go around the pipes?

They should end up at the bend where the pipes cross the rear axle after slipping over the pipes. These are used to insulate the pipe so that excess heat does not escape and melt your fuel line and electrical cables. Install them first, then make any necessary adjustments after the pipes are in place.

Is this a simple bolt-on or will an xpipe be required?

For a 2011-2014 GT, this is a direct bolt-on replacement.

Is this compatible with a Flowmaster axle back exhaust system?

I have a set of Flowmaster Outlaws, and these fit perfectly.

There is no need for welding because the fit is perfect. It was installed by a mustang shop in the area. Borla's type axelback and the sound are a perfect match. My s types sounded better and more aggressive as a result of the increased volume.
